The dilemma: the incentives for the doctor, including the time crunch due to cost controls.
My medical care is passive, not proactive. It is reactive, if something comes up, but there is only minimally dealing with the problem.
It's like "well, you're old and this things happen normally, so we'll just medicate, manage any pain, and you just follow the instructions (which are minimal, such as "take the pill").
